1. Increase in Sum Assured
   
  Documents required:
 
a) For Whole Life/Endowment/Term products
 
Alteration Request Form
Personal statement (Declaration of health)
Medical examination may be required subject to terms and conditions
If there is an increase in premium, please;
 
  1. Submit a new auto-debit authorization form if there is an authority limit on the previous form
  2. Submit a new credit card enrolment form if you want to charge your payment on a new credit card
  3. Advise your bank on the increase of premium to be paid for payment via standing instruction
Before changes to the benefits of policy take effect, premium has to be paid up-to-date
   
b) For Investment linked products
 
Supplementary Proposal form
Medical examination may be required subject to terms and conditions
Other medical / financial requirements (e.g. declaration of health and sum assured applied for)
If there is an increase in premium, please;
