|2. You can contact your agent for the necessary requirements or you can obtain the information and claim form at this website. Alternatively, please call our Customer Line at 03 - 21160228 and our Customer Service Officers will be pleased to advise you on the Claim requirements and appropriate forms to complete.|

4. Complete the required forms.
